MAKE A CLAIM Learn How to Make a Claim Below:

Claims Deadline: All claims must be submitted to RWAM no later than 12 months from the date the expense was incurred. In the event of termination of coverage, claims incurred prior to the termination date must be submitted to RWAM within 3 months of the termination date.


The easiest way to make a claim is to use the RWAM Member Portal. However, you may still submit a claim via e-mail, fax or mail by following the instructions in the appropriate tab below. 

 

Once you have access to the portal, you can download the Plan Member app and submit claims easily using your phone.

 

Tips for Registering:

 

  1. Use either Google Chrome or Firefox
  2. Click "Register Now" to open the registration page
  3. Enter your first name the same way you have registered at school (you may or may not need your middle name)
  4. Your Group Number is 490002
  5. Your ID on the plan is your Student ID number

Once you've completed the form in full, click "Continue", your User ID will be displayed on the screen as a 12 digit number - WRITE THIS DOWN.  You will then be prompted to have your password e-mailed to you. Once you get the e-mail you're in!

Making a Claim on the RWAM Plan Member Portal:

You are able to submit claims for Drugs, Extended Health, Vision & Dental on the portal.

Once you've registered and logged on to the portal:

 

  1. Click "Submit a Claim" on the left hand menu
     
  2. Review your information to ensure it is correct
     
  3. Choose the type of claim:
    Health - all health related claims (encompasses drug, paramedical & vision)
    Drug - drug claims only
    Paramedical - practitioners, medical equipment & supplies, etc.
    Vision - eye exams, frames, lenses & contact lenses
    Dental - dental claims, except dental accidents

     
  4. Complete the remaining fields
     
  5. Attach a clear scan or photo of your receipt & click Submit Claim

After you've submitted the claim, it will list as PENDING on the portal, when the claim is pulled for processing, it will not appear on the portal again until it is processed.

 

Checking your Claim Status

If you've submitted your claim online through the portal, or using the RWAM member app, you can check your claim status by clicking "Claims" top menu bar, then performing a "Claims Search".  Your claim will list as pending, or it will show an amount that has been reimbursed.  If you cannot see your claim anymore, that means it's been pulled for processing.  Check back again in 24 hours and it should re-appear as processed.

 

It takes 3-5 business days to process a claim starting the first business day after submitting your claim.

If you do not wish to submit on the RWAM Member Portal and want to submit a claim manually for an expense for Prescription Drugs, Extended Health (i.e. practitioner appointment, medical equipment & supplies, orthotics, etc.), or Vision you can follow this process:
 

  1. Download and complete the Health Claim Form
  2. Attach copies of your receipts 
  3. Submit your claim to RWAM using one of the following methods:

When you submit claims manually, they will not appear on the RWAM member portal until they have been processed.

It takes 3-5 business days to submit a claim starting the next business day after submission for faxed or e-mailed claims, and the date of receipt for mailed claims.

If your dentist is unable to submit your claim electronically using the information on the Drug & Dental Benefit Card, or you do not wish to use the RWAM member portal to submit your claim, you can submit a claim manually following one of the processes below:


Using Standard Dental Claim Form from Dental Office:

  1. Complete Part 1 if not already completed by your dental office
    Do Not sign the assignment of benefits section or the claim will not be processed correctly.

     
  2. Complete Part 2 - Employee/Plan Member Section

    Group Policy #: 490002
    Employer: Wilfrid Laurier University Students' Union
    Name of Insuring Agency or Plan: RWAM Insurance Administrators
    Certificate #: Your Student ID number

     
  3. Complete Part 3 - Patient Information

    Relationship to Employee/Plan Member: Self (unless for dependent)

     
  4. Complete Remaining Questions


Completing Form using Student VIP Dental Claim Form:

  1. Give the Student VIP Dental Claim Form to dental office for completion
    Do Not sign the assignment of benefits section or the claim will not be processed correctly.

     
  2. Complete Part 3 - Patient Information

    Relationship to Employee/Plan Member: Self (unless for dependent)

     
  3. Complete Remaining Questions

In the event that you have to pay for your medical emergency out of pocket, you will need to submit a Travel Claim Form to be reimbursed.  Download and complete the form and submit it by mail to:

What is an Accident? 

An accident means an occurrence due to external, violent, sudden, fortuitous causes beyond the insured's control, which must occur while the student is insured under this policy.


Complete the Accident Claim Form and attach all invoices and receipts incurred as a result of the accident

Accident claims will first be considered under your Extended Health or Dental benefits, any balances will then be considered under the Student Accident benefits.


Expenses for Accidents will be Considered if:

  • Within 30 days from the date of the accident the student obtains medical treatment in Canada
  • Expenses are recommended by a legally qualified physician or dentist
  • Expenses are incurred in Canada
  • Expenses are incurred within 52 weeks of the date of the accident causing injury
